Sub-acute Rehab and Skilled Nursing Facility
Skilled Nursing Facilities (SNFs) offer short-term care for patients recovering from an illness or injury, as well as long-term care for patients who require 24-hour care in a residential setting
.
Typically, patients who require short-term SNF care are patients who don't require to be in the hospital but also are not strong and independant enough to be at home. At the SNF care team is to provide the appropriate therapeutic interventions in order to facilitate recovery and strengthening.
Skilled nursing facilities offer a wide range of rehabilitation programs, including services for musculoskeletal, neurological, complex medical, amputee, stroke and pulmonary conditions.
